IRRITABLE BOWEL SYNDROME: DEFINITION: The cause of irritable bowel syndrome is unknown. Although often called "colitis", it is not an infection or inflammatory condition. SYMPTOMS: Symtoms vary, but can include periodic abdominal cramping, migratory abdominal pains, diarrhea, or constipation. Commonly, a few days of constipation is followed by loose stools, then constipation begins again. DIAGNOSITIC TESTING: There is no specific test for irritable bowel syndrome. The disease is diagnosed by 1) history and exam findings, and 2) finding no evidence of other disease. It is common for physicians to order a barium enema xray or do a colonscope to get a clear cut understanding of this disorder. THERAPY: Irritable bowel syndrome is treated by making the stool softer and bulkier. Regular meals which include plenty of soluble fiber help. Avoid foods which provoke cramping. Stool "bulking agents" such as metamucil help. Occasional flare-ups are expected. IF PROBLEMS: Call the physician if symptoms worsen -- such as severe or constant abdominal pain, fever, blood in the stool, increasing constipation, or more frequent or severe diarrhea.
